Ancient Greek medicine presents a paradox: it was more empirically rigorous than anything that came before it, while remaining embedded in theoretical frameworks that we now know to be completely wrong. The Hippocratic tradition represented a genuine break from purely supernatural explanations of disease, and yet its replacement — the four humors — was itself a theoretical system with no sound empirical foundation.

The Hippocratic case histories preserved in the Epidemics are genuinely empirical: descriptions of specific patients, their symptoms, the course of their illness, and the outcome. They don't moralize, they don't invoke supernatural causes, and they observe with what would be recognizable clinical method. The famous prognosis-over-cure emphasis — predicting how an illness would progress rather than claiming to treat it — reflects an honest assessment of what ancient medicine could actually do.

Galen's system, built on Hippocrates and extended into a comprehensive physiological theory, dominated Western medicine until the 17th century. The humoral theory (blood, phlegm, yellow bile, black bile) was wrong but not useless: it provided a coherent framework for understanding why some treatments worked (when they worked by accident, the humoral theory could accommodate the observation). The real problem with Galenic medicine was the authority it acquired, which prevented experimental revision.

Ancient surgery was more effective than ancient internal medicine because the problems it addressed (wounds, fractures, cataracts) were more tractable by empirical methods. Archaeological evidence of healed surgical interventions — trephination, limb amputation — shows that ancient surgeons could produce outcomes that modern surgeons would recognize as reasonable.
